Instead of billing insurance for every visit or service, we charge a monthly subscription. This gives you direct access to your provider without the red tape.

Unlimited office visits with no co-pays

Same-day or next-day appointments

Direct communication via phone, text, or email

Annual physical exams and wellness checks

Chronic disease management (e.g., diabetes, high blood pressure)

Membership Plan

Age 4-18 years old:
$49/month

Age 19-64 yeas old:
$89/month

Age 65 years old and older:
$99/month

Family Plan: $220/month
for 2 adults and 1 child
Each additional child $30

Single Parent Plan: $130/month
for 1 adult and 1 child
Each additional child $30


Enrollment fee:
$50 per person
Maximum $150 for a family that enroll at the same time.
Second timer re-enrollment: $100
Third timer re-enrollment: $200
